Planning your admission

Admission process is the same for both young and adult residents

Referral process

We offer introductory visits to prospective tenants and avoid unplanned admissions except in a case of emergency.

Once we receive a referral, we arrange to meet the potential resident so that we can assess their needs. We will investigate all area of daily life so that we can determine if we are able to fully meet their needs. If the by the end of the assessment we determine that we can meet potential resident’s needs, we will invite them to visit the property so that they can have the opportunity to look around, meet other tenants on the property and ask questions or seek clarification about anything they are unsure of.

If the potential resident likes our service, there is an appropriate vacancy for them and funding for their placement has been approved, a transition plan is then agreed with the individual and others involved in their care. Transitioning can include both day and overnight visits to help the new resident to familiarize with their new accommodation so as to minimize the change impact, and smoothen the settling in process. After this and if the potential resident is happy to move in, an admission date will be agreed. We tell our potential residents what we expect of them, and what they should expect from us (our policies and house guidelines) way in advance so that they can make informed decision.

In the case of emergency admission request, an initial assessment will be completed within 2 working days of admission. If the we can offer a suitable placement, and funding is confirmed, we will inform the new resident of all-key aspects, procedures and routines of the property within two days of admission. Prior to admission, a review plan will be agreed with the potential resident and their care team and contingency plan confirmed as to what actions will be taken if the placement becomes unsustainable. The placement will be formally reviewed in sixth week and a decision made on whether support can be continued.

Visiting our home

We offer introductory visits to prospective residents and avoid unplanned admissions except in cases of emergency. We understand the challenges that changes pause to people with learning difficulties which is why our transition process is gradual. Several visits are made prior to admission; introduction/familiarization to environment and members of staff whilst observing needs and necessary adaption. A resident spends hour for starters, then days, a few nights before they move in permanently.

You will be met by caring, kind and warm staff who will see to it that you have great visiting experience, and of course meals are on us.

Visits are arranged way in advance considering that we accommodate individuals with learning challenges thus, actions have to be put into place before visitors come in.

We ensure that all visitors sign in and out of the building and we restrict access to tenant’s rooms without prior consent of the Registered Manager, for example tasks such as routine maintenance and health and safety checks are conducted under specific arrangements.

In the face of Covid-19, we strictly adhere to guidelines and may lock down the home if need be

Accommodation

We recognise that prospective tenants should have the opportunity to choose a home that suits their needs. To facilitate that choice, we do the following:

We ensure that prospective residents undergo thorough needs assessment before a decision on admission is taken, and demonstrate that we are confident we able to meet the potential resident’s needs

We provide detailed information on the service by through a service user guide.

Our Tenancy Agreement clearly specify what we expect from the resident, and what they should expect from us.

Residents can be guided to discontinue their residence if they feel unhappy in our home. This has never occurred before, but it is an available option.
